The prospectus is on one side of the leaf, headed in capitals 'Money obtained by the sale of war savings certificates will be used by the Commonwealth for war purposes only.' The reverse carries the 'Form of Application for Commonwealth of Australia 4 1/2 Per Cent. 3 Years' War Savings Certificate.' The form has been completed and signed by Lieutenant J. Moss, at Selborne Chambers, Howard Street, Perth, with an application for £875.
