He begins by stating that he 'constantly' sees 'patients in London for the ordinary Consultation Fee of 2 Gs.' Because of the 'lamented death of a relative at a distance', Conolly is 'unable to name an earlier time for having the pleasure to meet you than Monday next, the 21st., at 3 o'Clock'. TWO: 2pp, 12mo.
